No way to toss off a salad!

Here are examples of three salads with critical errors in preparation. Can you see what might have gone wrong?
The salad to the left isn't filled, vegetables don't appear to be fully mixed, and the cucumber slices aren't placed evenly.
In the center, vegetable are on the large side; cucumber slices are too thick and there aren't enough of them to hold up in the mold.
On the right, cucumber slices are placed unevenly. Also, although difficult to spot on line, each of these plates had spots of vinaigrette around the edges.
